Important. This is a care-navigation prototype, not a diagnosis. If someone is severely unwell, deteriorating, unconscious, having severe breathing difficulty, a seizure or major bleeding, seek emergency help now.

PUBLIC CARE NAVIGATION

Where should I seek care?

Start with emergency warning signs. If none are present, answer a few simple questions to guide the next appropriate level of care.

About the problem

This does not diagnose the condition. It helps determine how urgently assessment may be needed.

Safety net: If symptoms worsen, new warning signs appear, you are worried the person is seriously unwell, or appropriate clinic care is not accessible, seek urgent in-person medical assessment. Do not use this tool to delay emergency care.
Safety first. If the patient appears critically ill, resuscitate and escalate immediately. This tool supports, but never replaces, clinical judgement.

CLINICAL • TRIAGE AWAY SUPPORT

Could this non-emergency patient be redirected to primary care?

This is a policy checklist after secondary triage — not an automatic discharge decision.

KKM policy reference: Emergency Medicine & Trauma Services Policy states that non-emergency cases may be triaged away; documentation and secondary triage with vital signs are required; triage away must not occur from primary triage; and children below 12 years should not be triaged away. Local hospital SOP may add stricter requirements. Hospital Kluster Melaka, for example, requires patient agreement and directions to the nearest outpatient clinic, and accepts patients who decline redirection, have disability, or have difficulty accessing the clinic.

Prototype rule: Aulawiyah only indicates “eligible for clinician consideration.” Final disposition remains subject to the current approved KKM/local ED policy and responsible clinician/triage officer.
